Essential Smile Plan
Preventive care and basic maintenance
Two routine cleanings per year
One Annual exam per year
One emergency exam per year (problem-focused, excludes treatment).
One set of bitewing X-rays annually
One full-mouth X-ray (FMX) every 5 years, or as clinically indicated.
One fluoride treatment per year (based on clinical need).
One oral cancer screening per year using Velscope (adjunctive tool).

PerioCare Plus Plan
Periodontal maintenance and advanced care
Three periodontal maintenance visits per year (By hygienist)
One exam per year
One emergency exam per year (problem-focused, excludes treatment).
One full-mouth periodontal charting per year
One set of bitewing X-rays annually
One full-mouth X-ray (FMX) every 5 years, or as clinically indicated.
One fluoride treatment per year (based on clinical need).
One oral cancer screening per year using Velscope (adjunctive tool).

Priority scheduling for periodontal visits
Our In House Plan will provide coverage for one year from the date of enrollment and if the benefits are not used within the allotted time the benefits will be forfeited. It is the patient’s responsibility to schedule the necessary appointments within the allotted time period. The fees in our ”Membership Plan” apply to services performed by Dr. Andrea Cardoso only. Occasionally, the services of a specialist are required and these fees will not apply. Fees are subject to change without notice at any given time. This is not insurance; by joining this plan, patient receives discounted fees for which he/she is responsible at the services are rendered.
TERMS AND CONDITIONS
No insurance required.
Cannot be combined with other discounts or insurance.
Non-refundable once activated. (Refunds may be considered only in cases of documented relocation or death.)
Services must be used within the membership year
Membership auto-renews annually unless canceled in writing at least 30 days prior to renewal date.
This is not dental insurance. Membership benefits are only valid at Atlantic Coast Dentistry and are non-transferable.
EXCLUSIONS (What’s not included in discount)
Cosmetic procedures (e.g., includes but not limited to whitening, veneers, Botox, etc)
Lab fees for crowns, bridges, dentures
Specialist referrals or outside services
Missed appointment / cancellation fees
