Cannot be combined with other discounts or insurance.
Non-refundable once activated. (Refunds may be considered only in cases of documented relocation or death.)
Services must be used within the membership year
Membership auto-renews annually unless canceled in writing at least 30 days prior to renewal date.
This is not dental insurance. Membership benefits are only valid at Atlantic Coast Dentistry and are non-transferable.
